5676 Clinical Supervision Round 3 REALD Addendum: Following the Effective Date of a grant agreement until final data has been submitted with respect to the term of the grant agreement, REALD (Race Ethnicity Language and Disability) data shall be voluntarily collected from Clinical Supervision Round 3 individual beneficiaries (i.e., individuals who receive grant funding) in a form prescribed by OHA. Additionally: 1. REALD information should not be requested from an individual more than one occurrence within a 12-month period. 2. Funding may not be conditional for any individual providing REALD data but grant recipients must request that beneficiaries respond to the REALD questions using any instructions and templates for collecting such information as provided by OHA.
